Evaluation Forms

Please find all active six month and final evaluation forms posted here. Should you have any questions about completing the forms, please contact Jennifer Kuntch, Program Officer for Community Investment at jkuntch@tfec.org. If your project is 100% complete by the six month mark, please utilize and submit the Final Evaluation Form and your file will be considered complete and closed. Please submit these documents by the deadlines listed in your grant agreement and award letter.
