On behalf of myself and/or my organization, I authorize TFEC, its assigns and transferees to copyright, use, and publish the same in print and/or electronically. I agree that TFEC may use such media for any lawful purpose, including but not limited to, such purposes as publicity, illustration, advertising, web content, and social media (i.e. Facebook, Twitter, YouTube, etc.).

    I have received necessary media release forms from any and all individuals pictured, videoed, speaking and/or who provided testimonials.
